What is the normal size of the prostate?

In men between 21 and 30, the normal prostate weighs about 20 grams, give or take 6, and that weight stays roughly the same as a man ages, unless BPH develops [1]. Doctors usually measure the prostate by ultrasound, in millilitres. For the prostate, 1 millilitre is close to 1 gram.

When BPH develops, the prostate grows. In one analysis, a prostate with BPH weighed 33 grams on average, but they varied a lot, and only about 4 in 100 men over 70 had one larger than 100 grams [1]. Size alone does not decide whether you need treatment. Your symptoms matter most.

What is a normal PSA level by age for Indian men?

PSA (prostate-specific antigen) is a protein made by the prostate, and a blood test measures it. The upper limit of normal goes up with age, because the prostate grows. Two Indian studies show that Indian men's PSA ranges are lower than those used for men in the West, and the studies are not identical, because the men studied were different.

What does a high PSA mean?

A high PSA is a signal to check, not a diagnosis. In the Gujarat study, about 89 in 100 men had a PSA under 4, about 9 in 100 had one between 4 and 10, and about 2 in 100 had over 10. PSA also rose with prostate size [2]. An enlarged prostate can raise PSA even when there is no cancer. Infection can too.

That is why a raised PSA is followed by a doctor's examination, and sometimes an MRI or biopsy, so that cancer is ruled out properly.
