Thyroid nodule ablation is an ultrasound-guided technique in which a fine electrode is placed into a benign thyroid nodule and radiofrequency or microwave energy heats the tissue precisely. The nodule shrinks over months and symptoms improve, without removing the thyroid gland.

Benign nodules confirmed on two Bethesda category II biopsies, autonomously functioning (toxic) nodules, and, in specialist hands, selected recurrent papillary thyroid cancer lymph nodes. Suspicious or primary thyroid cancer is not treated with ablation as a first-line option.

The recurrent laryngeal nerve is carefully mapped on ultrasound and hydrodissection with a saline barrier keeps energy away from the nerve. Voice change, when it occurs, is usually mild and temporary and settles within weeks.

Most patients do not need thyroid hormone tablets because functional healthy thyroid tissue is preserved. TSH is checked at 1, 3 and 6 months, and levothyroxine is only started if hypothyroidism develops, which is uncommon.

Published data from the Korean Society of Thyroid Radiology and Cleveland Clinic show volume reduction of 50-90% within 6-12 months, with continued shrinkage over the following year.

The ablation session usually lasts 30-60 minutes, depending on the size of the nodule. You are observed for a few hours after the procedure and discharged the same day.

Local anaesthesia and cool saline flushing keep the procedure comfortable. Mild neck ache for 24-48 hours after the procedure is usual and responds to simple pain medicine.
