  • Is a bigger thyroid nodule more dangerous? Not necessarily. Thyroid nodules are not judged by size alone. On ultrasound, we also look at features such as the nodule's: • Shape • Margins • Composition • Echogenicity • Calcifications / echogenic foci These findings help determine the TI-RADS category. Based on the ultrasound risk pattern, nodule size and clinical context, FNAC/biopsy may be recommended. FNAC then helps determine whether the nodule is benign, malignant or indeterminate. In selected indeterminate cases, such as some Bethesda III nodules, molecular testing may provide additional information. So don't assume: Small = Safe ❌ Large = Dangerous ❌ The complete picture matters.   • TI-RADS 4 on your thyroid ultrasound report? It does NOT automatically mean cancer. TI-RADS 4 means the nodule has ultrasound features that warrant closer evaluation. The next question isn't immediately: “Do I need surgery?” It's: “Does this nodule need FNAC/biopsy, and what does the cytology show?” If further evaluation confirms that the nodule is benign, surgery may not always be necessary. For appropriately selected symptomatic or large benign thyroid nodules, thermal ablation such as microwave ablation (MWA) may be an option while preserving the thyroid. So if your report says TI-RADS 4: Don't panic. Don't ignore it either.   • Bethesda 3 on your thyroid FNAC report — does that mean cancer? Not necessarily. Bethesda III (AUS) is an indeterminate cytology result. It doesn't clearly establish that the thyroid nodule is benign, but it also doesn't confirm cancer. So a Bethesda 3 report by itself shouldn't automatically mean: ❌ Immediate surgery ❌ Immediate ablation ❌ Ignoring the nodule altogether Depending on the individual case, further evaluation may include repeat assessment and/or molecular testing such as NGS, alongside the ultrasound pattern and complete clinical picture. If evaluation suggests a higher malignancy risk, surgery may be appropriate. If the overall assessment is reassuring, some patients may be suitable for active surveillance. And when a suitably evaluated low-risk nodule still requires treatment because of its size or symptoms, thermal ablation may be considered in carefully selected cases. The decision isn't made from one line in an FNAC report. FNAC + Ultrasound + Molecular findings when appropriate + Clinical factors = the complete picture. They often mean smaller access, less pain, lower blood loss and faster recovery than open surgery, but safety depends on your diagnosis, scan findings and overall health.

Most are done with local anaesthesia and, when needed, light sedation. You may feel pressure or mild soreness afterwards, but severe pain is not expected.

No. An interventional radiologist is a doctor trained in radiology who treats conditions through a needle-sized puncture using live imaging, instead of an open operation. If your scans show that surgery is the better option, Dr. Nagateja Bonala will tell you so.

In many patients, yes. Uterine fibroid embolization, laser or glue treatment for varicose veins and varicocele embolization are done through a small access point without open surgery. Whether they suit you is decided after examination and a scan review.
