Before you agree to varicocele surgery

The usual advice for a painful varicocele, or one affecting fertility, is an operation called varicocelectomy, which ties off the enlarged veins through an incision, often under spinal or general anaesthesia.

Many men are not told that the same vein can be closed without an operation. In embolization, an interventional radiologist passes a thin catheter from a vein in the neck or groin to the leaking testicular vein and closes it with tiny coils or a sclerosant. Blood is redirected through healthy veins [1, 2]. There is no cut in the scrotum, no stitches and no general anaesthesia.

What the research shows

A systematic review and meta-analysis of 16 studies with 2,138 patients compared endovascular and surgical treatment for varicocele. Recurrence and subsequent pregnancy rates were similar, and adverse events were lower in patients who had endovascular treatment (relative risk 0.63, 95% CI 0.42-0.93) [3]. The authors concluded that endovascular treatment gives similar results with fewer adverse events.
